To request/view a certificate of insurance, proof of insurance, Waiver of subrogation, or other lines of insurance, click ‘login’ on the OCMI website.

Log in to the client portal with your credentials.

To view your previously requested certificates and their statuses, select the relevant previous request.

To request a certificate, follow these steps:

1. Navigate to the certificate request module located in the left sidebar.

2. To initiate a new request, click on "Create Request" in the top right corner.

3. In the drop-down menu, choose the type of certificate you wish to request.

4. Make sure to fill out all the required fields in the form, as they contain essential information needed for issuing the certificate.

5. After reviewing your information, accept the acknowledgment and click "Submit" to complete your request.
